Forum 1975 with plain white cover and (complete?) set of five title inserts which are printed in green ink on grey, tan, lime, white, and lemon paper. Blue GLC Great Live Concerts labels on both sides. Cover retains its original shrink wrap. Slight warp not affecting play.
