Late 1970s/Early 1980s 2LP set recorded live at Cow Palace in San Francisco, CA on July 15, 1975. Includes plain white cover and title insert (black ink on white paper). Cover retains its original shrink wrap. Some faint yellowing along opening where shrink wrap has been pulled back. Also includes a 3-1/2" x 5-1/2" index card with handwritten details by someone involved in the manufacture of this release (states 1000 copies pressed, all have same insert, no other variations exist).
